A medical home is a model that has been in existence since the early 1960s. The model was started initially as a way to offer specialized care for the children who had chronic diseases, with the nurses being at the center of this health policy. Over the years, the concept has massively evolved and gained popularity in the health policy circles. It has grown to include not only the pediatrics but also the elderly and any other patients in need of medical care (Keeling & Lewenson, 2013). However, of key concern has been the constant debate on who actually plays what role in the medical home settings.
