Numerous government, scholarly, and clinical studies and reports have indicated that actions that clinicians take after a patient has been discharged from hospital determines the chances of for patient’s readmissions. Currently, there are ongoing initiatives that have proved fruitful in terms of reducing the possibility of readmission after discharging. Clinicians have the mandate of providing health care in the most professional and best knowledge possible. Making proper post-discharge measures that involves assessing the continuity and efficiency of medical and/or physical care could help mitigate the possibilities of hospital readmission, solutions that my clinical questions can help provide for the current and future care practice.
