Specially trained registered nurses have the mandate to conduct post-discharge measures in order to ensure that patients undergo the most appropriate healing process in transitioning to their home care. According to Mistiaen and Poot (2006), a proper post-discharge nursing care requires the nursing clinicians to offer a follow-up support in helping patients deal with problems associated with emotional issues, difficult in handling daily living activities, knowledge deficits such as dealing with insufficient knowledge in understanding advice or symptoms, informational needs, uncertainty and anxiety, and insufficient help. Fundamentally, the quality of post-discharge nursing care has been linked with the readmission rates, as it is evident that some readmissions are because of poor follow-up nursing procedures, particularly within the United States government’s estimated 30-day period after a patient has been discharged. Little research has been done to present some of the follow-ups capable of improving the post-discharge nursing care to help reduce the rate of readmissions, a limitation that requires extensive studies in finding out ways to mitigate the issue.
