Your department is having a pot luck Holiday luncheon. Create aform for collecting information about what food items people willbring. The form could collect the employees’ name and teamidentifier. The Team name should be a drop down list with at leastfour options. Use radio buttons to determine if the employee willbring an entree, side dish, dessert, or something else (likedrinks, bread, paper goods, contribute cash, etc.). When a radiobutton is selected, the appropriate field(s) will appear to collectthe information.
